Chronic Disease Management

A GP management plan (GPMP) involves your Doctor developing a plan to better manage your chronic disease or condition. The plan covers your health problems and assists in co-ordinating your care with other health professionals.

Eligible people have a chronic medical condition that has been present or is likely to be present for 6 months or longer, for example Diabetes, Asthma, Arthritis, Osteoporosis, Ischaemic and Heart Disease.

After preparing the GPMP, your Doctor may identify that you could benefit from the input of other health care providers, and will recommend a Team Care Arrangements (TCA). Then with your consent the GP will ask the relevant health providers to form a team and work together in providing care. The health providers may be:, Optometrists, Podiatrists, Diabetes Educators, Exercise Physiologists, Mental Health workers, Occupational Therapists, Physiotherapists, Dieticians, Speech Pathologists and Chiropractors.  People with a GPMP and TCA are eligible for 5 Medicare rebatable services per calendar year provided by health care professionals.
